Haggerty Arbitrations utilizes the following two forms which may be accessed by clicking the following links:

Once the parties have agreed that Haggerty Arbitrations shall serve as the arbitrator in their case, their attorneys should forward the signed originals of the Arbitration Service Contract and each of the parties' separate responses to the Arbitration Questionnaire (along with a copy of their pre-incident contractual arbitration clause or post-incident agreement/stipulation to arbitrate) by U.S. mail or overnight delivery to: Haggerty Arbitrations, 1400 Coleman Ave., Suite C-21, Santa Clara, CA 95050. (A cover letter indicating that all of the parties or their attorneys have been provided with copies of these documents should also be included.)
